Capitol County Children’s Collaborative is the New Unified Care Management of Mercer County!!
An exciting change for children, families, service providers and the community, combining Youth Case Management and Care Management services, the new Unified Care Management model will allow children and families to make a smooth transition between moderate and high intensity services and should help speed linkage to community-based sustainable services.
There will be some changes to the look of documents and service plans, but we expect the transition to be as close to invisible as possible for children, families and the Child Family Team.
As Capitol County Children’s Collaborative expands to provide service to a greater number of children and families, we will be bringing new staff members on board. We are working to increase our number of formal service providers and sustainable community supports as well.
Capitol County Children’s Collaborative will host a series of open houses and affinity groups to discuss Unified Care Management in more detail. Staff will also be available to answer questions by phone and email. More information about these resources will be made available soon and all updates will be posted on www.MercerResourceNet.org and the CCCC website, www.capitolkids.org.
