Serve youth/families that are experiencing emotional, behavioral, substance use disorders, intellectual and/or developmental disabilities and chronic medical health challenges. Our employees serves as Care Managers for youth enrolled with our program. Our Nurses and Health and Wellness Educators provide additional support for youth who meet qualifying medical conditions and are enrolled with our Behavioral Health home. There is no charge for CCCC's services.

Access 24 hours a day and 7 days a week. During our normal business hours of 9:00am-5:00pm, you can reach our staff by calling our main office at 609-584-0888. If you are an enrolled youth or family our are working with one of our enrolled families, you can reach us outside of our normal business hours at 609-584-0888 and press 1 if it's an emergency..

Facilitate, coordinate and work within child-family teams to develop individualized service plans (ISP) and connect to community sustainable resources.Our Care Managers conduct face to face visits optimally two times a month. And our Behavioral Health Home staff, who provide additional support for youth with qualified chronic medical conditions, visit with youth and families on an as needed basis but not less than once per month.

Keep children in their own homes, schools and in their community. We provide direct, face-to-face care coordination and wraparound care planning for children and their families with the most complex needs (mental health, behavioral, developmental delays, substance use disorder, and chronic medical challenges) and are responsible for facilitating access to a full range of treatment and support service.

Care Mangers assist youth and families with referrals to programs that are customized to each youth’s needs but may include mentoring and therapeutic services. BHH staff provide assistance with medical and wellness resources.
