What We Do

We provide person-centered care management and wrap around services to assist the family in identifying their vision of success. Through the youth and family’s active participation, we identify strengths and needs to help find, create, and develop strategies that will support this vision through the development of an individualized service plan (ISP). Using informal and formal supports, a Child Family Team (CFT) is built to assist with linkage to sustainable services and resources within the community to achieve success and thrive independently.

When a child is struggling with behavioral health challenges, developmental needs, or complex medical conditions, the system can feel overwhelming. Our care managers work face to face with your family to build a plan, find the right providers, and make sure nothing falls through the cracks.

Our services are free. There is no cost to families.

What to Expect After You Call PerformCare

After your call, you will be connected to the right services for your child's needs. If CCCC is the right fit, you will be matched with a care manager who reaches out to set up a first meeting. There is no cost to your family at any step.
