CCCC works with four kinds of partners across Mercer County. Each section below explains who it's for, why the partnership matters for kids and families, and how to get started.

CCCC partners with people and organizations who share our mission to Engage, Educate, and Empower children and families - coordination community and support that helps families stay healthy, connected, and independent.

Empower

Service Providers

For approved agencies whose staff deliver hands-on services to Mercer County children and families.

Who This is For

Agencies approved through New Jersey's Children's System of Care (CSOC) that have in-person staff serving Mercer County. Working with us means setting up a formal partnership agreement - sometimes called a Memorandum of Understanding, or MOU.

What These Partners Provide

Services like in-home therapy, one-on-one behavioral support, mentoring and parent mentoring, structured parenting programs, and clinical evaluations - most of it in the family's home or community, not an office. (Limited video sessions are available for specific situations.)

Good To Know

  • We also partner on flexible, enrichment-style supports - social-skills groups, recreation, fitness, and creative or expressive therapies.
  • Services are built around each family's needs and goals - not a one-size-fits-all program.

Why This Partnership Matters

When a family's Child Family Team builds a care plan, these are the partners who carry it out - in the living room, not an office. Strong providers mean a child gets the right help close to home, which is the heart of wraparound care.

Community Organizations

For local programs that give kids and families places to grow, belong, and have fun.

Who This Is For

Organizations offering community-based programming — mentoring, social-skills and recreation groups, counseling, intensive outpatient and day-treatment programs, substance-use support, and after-school or summer programs.

How We Work Together

  • We host information sessions so our care managers know your program.
  • We invite you to list your services on MercerResourceNet and to join us at community events.
  • We offer short-term (90-day) transition contracts to help families step down from intensive care to ongoing community services.
  • We co-plan events like Trunk or Treat, Toy for Every Child, and Spring Family Fest — and welcome partners who can enrich our Care Manager's skills.

School districts and personnel — nurses, counselors, child study teams, and administrators — partnering with us to connect students and families to the right support.

How We Work Together

  • Staff workshops on the children's system of care and how care management works.
  • Parent workshops that help families understand and navigate support.
  • Tabling at school events, wellness fairs, and back-to-school nights.

Why This Partnership Matters

Children spend most of their day in school. When educators understand the children's system of care, they can spot needs early and become full partners on a child's team — so support follows the child between home and the classroom.

System Stewardship

CIACC - The County's Children's Council

For agencies and advocates helping coordinate services across Mercer County. (CIACC = Children's Interagency Coordinating Council.)

How to Get Involved

The full council meets at Mercer Care Management Organization on the 4th Thursday of each month at 9:30 AM (with a summer break). In-person attendance is encouraged, and a video option is available.

Subcommittees

Intellectual & Developmental Disabilities

Closing service gaps and improving access for children with intellectual and developmental disabilities.

Meets: 2nd Tuesday monthly, 1:00 PM (virtual).

Education Partnership

Connecting schools, behavioral-health, and child-protective systems around children's well-being.

Meets: 3rd Wednesday monthly, 10:00 AM (virtual).

Early Childhood (Ages 0-5)

Improving coordination and advancing fair, early support for our youngest children.

Meets: Meets: 3rd Wednesday monthly, 11am (virtual)

No single agency holds a child's whole world. CIACC brings county systems — behavioral health, schools, child welfare, and developmental services — to one table, so families don't fall through the cracks between them and children get the least disruptive care possible.
